Community Health Partners (CHP) has been in existence
for 7 years. Accomplishments during this time include:
The
organization has grown 15 – 20% annually, now employing
over 90 staff members at its four sites.
Active
participation in disease management, emphasis on patient-centered
management of diabetes, cardiovascular disease, and depression. CHP
significantly exceeds national management goals for our patients
with these diseases.
Recognized
nationally as the GOLD Standard in Health Literacy and Clear
Client Communication Strategies.

as Teachers program – CHP created
this early childhood stimulation program through parent support
and education in partnership with schools.
CHP/
School Collaboration Program: Designed to help prevent adolescents from dropping out
of school.
Community Health Outreach and Partnerships
Working with Women, Infants and Children (WIC) and the
public health departments to offer a full compliment of services.
Numerous
partnerships with area medical specialists to provide
health care expertise for 300 patients annually. In-school
and after school pediatric dental exams, working with
local schools and Head Start.
On-site
obstetrics through partnership with local OB/GYN practice.
On-site
services in the county jail and community
food distribution
sites.
Psychiatric
Care: Strong collaborations with area mental
health centers to fully serve needs of clients, on-site and
through
referral networks.
Outstanding
support from Bozeman Deaconess Health Services (BDHS) and
Livingston Memorial Hospital, discounted outpatient services.
Ongoing, significant direct support from Bozeman Deaconess in
the areas of facility and oral health access support.
Community
Health Partners has a strong history of program design innovation
and initiation, as well as successful collaboration. All
programs have evaluation components built into the design.
As the above
list indicates, CHP has acted as a catalyst in addressing a number
of community health issues, in addition to drawing on great community
strengths in Gallatin and Park Counties.
Yet,
there is still so much to be done, especially in the areas of
expanded access to excellent services in both Gallatin and Park
Counties.
